USE OF FACE COVERINGS POLICY
The Board of Education recognizes its responsibility relative to student, employee, vendor, and visitor health and safety.
The spread of certain communicable diseases, specifically those that are transmitted via respiratory droplets, may be limited by use of face coverings, including face masks and other face shields (including, but not limited to cloth/fabric face masks and face shields that wrap around the face and extend below the chin) when social distancing cannot be maintained.
In order to maintain a safe and healthy school environment, and reduce the potential spread of communicable disease, the Board of Education complies with all federal, state, and local legal mandates intended to protect the health and safety of the District’s students, employees, and
visitors, including requirements to implement and enforce the use of face coverings while on District property and/or during offsite school events and programming.
The Superintendent shall establish procedures and requirements regarding the use of face coverings by District students, staff, vendors, and visitors, in compliance with legal requirements and any orders issued by the Ohio Department of Health, local departments of health, the Ohio
Department of Education or other entities authorized to regulate health or education within the State. In developing such procedures and regulations, the Superintendent also will consider available medical science and recommendations set forth by the Centers for Disease Control and
Prevention, U.S. Department of Education, Ohio Department of Health and local departments of health. The Superintendent shall update the procedures and requirements regarding the use of face coverings as needed based on changes to the law, orders or recommendations from the above-named entities. Open Enrollment
OPEN ENROLLMENT
The Berkshire Local School District will permit the enrollment of students from any other Ohio school district. Students wishing to enroll in the Berkshire Local School District may do so in accordance of policies and regulations adopted by the Berkshire Board of Education as
pertaining to Policy 5113 Inter-District Open Enrollment.
Applications for Inter-District Open Enrollment must be submitted to the Board of Education office no later than September 1. One application must be submitted for each student that is applying for open enrollment.
Proof of residency is required to be submitted yearly. Approved documents that will be accepted for residency are a current lease/purchase agreement signed by all parties, utility bill (i.e., electric, gas, phone), mortgage document, or property tax statement. Application forms will be
placed on hold until current proof of residency is received.
Parents who wish their student(s) to continue to attend schools within the Berkshire Local School District must reapply annually. There is no guarantee enrollment will be granted each year.
Inter-district enrollment limits may be set by grade level or on a course-by-course basis; no inter-district application will be permitted if the enrollment of the grade level being requested exceeds appropriate limits as determined by the superintendent.
All approved open enrollments are in effect for the current school year only.
Applicants will be considered on a first-come, first-served basis (date/time of receipt) with an assurance that resident students will not be displaced. The following order for placement will be followed:
• Students who have parents who are employees in the District
• Students previously enrolled in the District (priority status deadline: March 29)
• New student applicant
Transportation for open enrolled students shall be the sole responsibility of their parents/guardians.
Ohio High School Athletic Association (OHSAA) rules will govern all athletic eligibility decisions. For additional information, contact the Athletic Director, Brian Hiscox, at brian.hiscox@berkshireschools.org.
Instances of truancy and/or misbehavior will be handled according to the Berkshire Local School Districts’ policies and procedures. Recommendations for discontinuing a student’s placement would be recommended by the Principal to the Superintendent who will render the final decision. Open enrollment applications will be accepted until September 1, 2024.

If parents are living with another family within the Berkshire Local School District, they must provide a residency affidavit. The parent/guardian fills out the top portion and the homeowner fills out the bottom section. The form must be notarized. Both parties will sign the form in the presence of the notary. Proof of residency, as listed above (i.e. current electric or gas bill; purchase agreement OR current signed rental/lease agreement) is also needed from the homeowner.

Minor Work Permit
APPLICATION FOR MINOR WORK PERMIT
State Law requires that all minors, persons under the age of 18, who are in paid employment must have a valid Work Permit during the school year, and 14- to 15-year olds must obtain age and school certificates for employment at all times of the year.
An application for a work permit must be completed prior to obtaining a work permit. Applications may be downloaded or picked up from the main office at Berkshire High School. The application is to be submitted in person to Mrs. Maryalice Horton, Secretary, located in the high school office.
PLEASE READ THESE INSTRUCTIONS CAREFULLY TO AVOID PROCESSING DELAYS
During the school year, Berkshire High Schools main office issues Work Permits for all minors who reside in the Berkshire Local School District. Work Permits are required for all minors age 14 through 17 years.
- All pages of the Work Permit form MUST be completed prior to submitting and approval:
- PAGE 1 - The Applicant, Parents and the Employer complete this form
- Employers must include Tax ID Number and expected days and hours.
- If a student is emancipated or withdrawing and is under 18 years old:
- PAGE 1 - The Applicant, Parents and the Employer complete this form

PROOF OF BIRTH & INITIAL APPLICATION
- Berkshire Local Schools students do not need to submit Proof of Birth with their initial Application. Our internal database has the required information.
- Non Berkshire Schools students MUST provide Proof of Birth with their initial Application.
- The State of Ohio accepts the following as proof of birth:
- Birth Certificate, Driver’s License, State ID, Baptismal Certificate, Passport, Citizenship Papers

PHYSICAL EXAMINATIONS ARE REQUIRED
- Physical exams are required and are good for one year. Your physician will need to complete and sign the required sections unless you have a current physical on file with the Berkshire Schools athletic department.
