New Student Registration for Grades 1-12:

 

Who can register?

Registering a new student in Berkshire Local School District must be done by a custodial parent who lives within the district’s boundaries.  A student cannot be registered by a parent or relative who does not have legal custody of the child.

 

Registration is a two-part process.  

  1. Part one: Complete the online registration

  2. Part two: Schedule an in-person appointment to complete the registration process.

 

Procedure for registration (select one of the following):

  • EXISTING BERKSHIRE FAMILIES registering a new student should log into their Infinite Campus Parent Portal account and click MORE>OLR & FORMS>2022-2023 New Student Registration.

 

  • NEW FAMILIES TO THE BERKSHIRE SCHOOL DISTRICT registering a new student for the first time and do not already have a student enrolled in the district, please click on the Online Registration link here.

 

For all registering families, gather the required items below.  Parents/Guardians will need to bring these documents to their scheduled registration appointment.

 

Registration Documents include…

 

  1. Complete online registration

  1. Original or Official Birth Certificate (copies will not be accepted)

  2. Proof of Residency (see below)

  3. Residency Affidavit/Verification Form (if applicable - see below)

  4. Custody or Legal Documents (if applicable) – Official court stamped/signed documents pertaining to family situations (i.e. guardianship, custody)

  5. Valid Driver’s License/State Photo ID (Parent)

  6. A current signed IEP and ETR, if applicable

  7. Current report card/transcript

  8. Consent for Records Release 

 

Proof of Residency - Acceptable Document List

  • Property tax statement

  • Purchase agreement OR current signed rental/lease agreement with beginning and ending dates and listing all occupants.

  • Current electric or gas bill showing name and physical address.

  • Building permit

  • Voter registration card

 

Residency Affidavit/Verification Form (Residency Verification Form)

 

If parents are living with another family within the Berkshire Local School District, they must provide a residency affidavit. The parent/guardian fills out the top portion and the homeowner fills out the bottom section. The form must be notarized. Both parties will sign the form in the presence of the notary. Proof of residency, as listed above (i.e. current electric or gas bill; purchase agreement OR current signed rental/lease agreement) is also needed from the homeowner.
